How to write a doctors note?

everytime i've gotten a doc's note. its been on a perscription med paper and its just said, "Please excuse Jason from (insert activity here), for he has (put excuse here). He will be able to return to (place for which you are avoiding) on (create return date here). Thank you!

of course this all has to be completely illegable and have lots of squigglies that mean nothing. because if it's too neat, they'll know its a fraud!!! haha good luck!

Very true. Remember, doctors have a million things to do. Neat hand writing is not one of them.

Wouldnt the workplace look for a header on the note that includes...oh i dunno...a name, address, and phone number of the clinic? The doctor notes I've seen don't resemble the ones your mom wrote for you to get out of 2nd grade early to go to the dentist.

Ugh, everybody in my department is like that. I think your friend should just go to work, because I would think the consequences fpr forging a doctors note would be rather severe.
